Chemotherapy scheduler for the FNO outpatient clinic. It allocates patients to days and times to avoid peak periods, and patients know exactly when to arrive.
At present, doctors book patients in for chemotherapy manually, one by one. One day the day-care centre is overcrowded; the next, the chairs stand empty. In the morning, patients wait for their check-up and to collect their medication from the pharmacy, so the chairs are free, whilst between 11 am and 1 pm it’s at its busiest. Patients sit in the waiting room from 7 am and have no idea when their turn will come.
Our app plans this for them.
What it can do
- Import treatment plan: the doctor uploads a Word document with the treatment schedule; the AI extracts the individual steps from it, and the app calculates how long the patient will be in the chair.
- Booking: for a new patient, the system suggests the three most suitable days and explains why. It monitors available slots, the number of nurses on duty and when the patient’s doctor is available.
- Week: with a single click, it spreads the workload from overloaded days across the whole week, always by no more than one day at a time.
- Day: an hourly schedule of treatment chairs and beds, with traffic-light indicators and real-time patient statuses.
- Simulator: say two nurses fall ill in the morning. The head nurse can immediately see which patients are affected and confirm the change with a single click.
- Patient: a website and apps for iPhone and Android showing the time they are due to arrive for chemotherapy. If there is a change, they receive an alert and an SMS notification.
- AI assistant: answers questions in Czech such as “Why is Mr Novák not until 12?” and is able to optimise the schedule on request.
How it works
The data is stored in InterSystems IRIS for Health as standard FHIR R4 resources. The schedule is calculated by an optimisation engine in Python (OR-Tools CP-SAT); the backend is NestJS and the web frontend is React. Everything runs on Azure. IRIS handles authentication and roles via OAuth 2.0 and OpenID Connect. Vector Search was also utilised within the IRIS and InterSystems Interoperability interfaces to connect to an external service for sending SMS messages.
The system plans time and capacity; decisions regarding treatment and dosages remain the responsibility of the doctor.

- AI používáme tam, kde opravdu pomáhá. Karty léčebných režimů ve Wordu jsou každá napsaná jinak, takže z nich model (Azure OpenAI) vytáhne strukturovaný rozpis kroků a délku na křesle pak spočítá náš kód. Lékař výsledek jen zkontroluje a potvrdí. Druhé použití je AI asistent, který česky odpovídá na otázky nad plánem a umí vytvořit dynamické uživatelské rozhraní pro práci se systémem (předvyplněný formulář pro vytvoření rezervace nebo změna kapacit na pracovišti). Samotné plánování dělá optimalizační algoritmus, protože je přesnější a jde vysvětlit.
- Impact
- Na stejných 674 aplikacích za 21 dní, se stejnými křesly a sestrami, náš plán snížil přesčas o 52 % (442 h na 210 h součtu hodin navíc u pacientů). Dny, kdy sestry nestíhají zahájit všechna kapání, klesly ze 6 na 0 a dnů s úplně plnou špičkou kolem poledne je 9 místo 15. Stacionář tedy zvládne stejný počet pacientů klidněji a bez nových křesel. Pacient nemusí sedět od rána v čekárně, protože ví, kdy přijít, a při změně dostane SMS. Protože stavíme na FHIR a IRIS, dá se systém napojit i na jiná onkologická centra.

- Yes — Všechna data jsou v InterSystems IRIS for Health na FHIR R4 serveru. Pacienti, termíny, léčebné plány, režimy i průběh kapání jsou standardní FHIR zdroje (Patient, Appointment, CarePlan, PlanDefinition, Encounter), vlastní databázi nemáme. IRIS nám slouží i jako OAuth 2.0 a OpenID Connect server pro přihlášení a role a vede audit přístupů. Mimo to nám IRIS pomáhá s Vector Searchem pro vyhledávání pacientů. Pro napojení externí služby pro odesílání SMS bylo využito InterSystems Interoperability.
